EV-maximizing index numbers (which is what almost everyone means when they refer to this) have nothing to do with bet spread. They are simply the strike point at which the Initial Bet Advantage (IBA) becomes more favourable for one action over another. They do however vary from one counting system to another.
To answer Aceside's other question. Because we are evaluating the value of each index using SCORE, we must therefore use optimal bet spreads and all of the other conditions for SCORE as defined in BJA3.
To further explain, we examine not only different rules sets and betting spreads but also play-all and back-counting scenarios. It should be clear that the sets of indices (we use the "Sporty 40" for back-counting and the "Nifty 50" for play-all) included in each array do, in fact, vary not only as to which are included but also as to their ultimate SCORE rankings and the magnitude of contribution (both dollar-wise and percentage-wise) of each play.

Because the vast majority of players use EV-maximizing indices, and because the original I18 and Fab4 are EV-maximizing, that's what we used in the book, except for a couple of key plays which we discuss in some detail.
The usual practical solution for those on a limited bankroll is to use more back-counting and we have that covered as well.
Of course RA indexes only deal with doubles and splits, correct? Am i wrong to play more cautiously and only double and split at a slightly higher index when max bets are on the line? Wouldnt this significantly lower your RoR?
As for the infamous 88vX i really like to stand on this hand at a slightly lower index if i had a maxbet on the line, but full EV on all other bets....

Yes, ra indices are effective for plays for which you are increasing variance at the expense of only a little gain in EV. Some doubles and splits definitely fall into this category. Adjusting your strike point away from the EV-maximizing crossover point gives the gain in EV a chance to catch up with the increased variance.
