The graph shows the EV of all available actions across a range of true counts allowing you to visually see where the indices are. We promised that the complete set would soon be available online, and now that promise has been fulfilled.

I assume that your visual guides are for Hole Card games only and always include some form of surrender?

Any word on the reprint of your Complete Guide to Index Play correcting the typos and omissions?

The book is now available from Amazon, and the one omission is corrected. Not aware of any typos.

The surrender indices are always included in the index charts in the book and in the graphs. If your game does not have surrender, you simply ignore them.

In the book we examined all of the above with and without surrender and at various levels of penetration. For the ES ENHC game, we examined ES, ES10 and no surrender.
