I count by the hand -- ploppies can make unexpected plays, so I wait until they've waved off or busted and evaluate the hand as a whole (certain cards cancel each other out, so counting by the hand is pretty easy once you get the hang of it).
The exception is when the dealer is showing an ace -- then I count the table as a whole, and if there is no dealer blackjack, I then count the new cards as they are dealt.
Counting each card as dealt on each round (not just when an ace is showing) can be pretty mentally exhausting, so counting at least by hand is a far better approach (as far as I'm concerned).
There are those who go further and count the table as a whole all the time. For me personally, I've found the larger swings in RC this results in can lead to my making occasional count errors. But that a limitation on my skill I recognize and certainly doesn't apply to all by any means.
