Suppose a cocktail waitress at a strip hotel like Caesar's were given a 5000 dollar chip as a tip. Would she have to pool it? Could she simply cash it in and take the money herself? Would she have to try to break it down into smaller units? Would the casino make her report the income? I'm wondering if it would be more a problem for someone than a windfall.

No problem for her. Cocktail waitress tips are not pooled. It is up to her to report her own income on her 1040. Cash it.

Tip pooling be damned, she pockets it immediately. Getting it broken down and cashed will be no problem for her.
