To answer your questions, we only covered double deck and six deck. We were constrained for space (336 pages with just those two!) and felt that these were the most relevant to today's environment. We did run some sims for 8-deck and noted in the book that the results were very similar to those for 6-deck.
The indices in the new book were newly generated by me in consultation with Don. In BJA3, Chapter 5, Don presented and ranked indices for the I18, Fab 4 and Catch 22 for a fixed set of rules (4-deck S17). He also ranked them according to contribution to EV using a fixed bet spread. In Chapter 10 he presented updated indices for those plays and our new index numbers are in agreement with those. He went on to present SCOREs for hundreds of games using those indices and a variety of bet spreads.
The new book presents hundreds of indices for the games mentioned above and ranks them according to contribution to SCORE for a wide variety of game conditions and bet spreads. In that sense, it is similar to BJA3 Chapter 10 except that we go beyond the I18, Fab 4 and Catch 22 and provide rankings up to the "Nifty 50" or "Sporty 40" depending on the game. This allows players to decide when enough is enough when it comes to learning indices. As you know from reading the book, there is also extensive commentary on the nature of and the interaction between many plays of interest.
Regardless, you should also own a copy of BJA3.
