You have options when there are table mates, such as wonging out in negative counts. If you have other info, for example, all the good cards are behind the cut card, or an abnormal distribution of intermediates, leave the table.
If you are heads up, and leave the table with dealer not shuffling - (assuming only 1 table available) don't take a whiz break, take a shit break, and let someone else go to the table and get those gems. If the crap starts towards the end the shoe, just grind it out.
If you have table mates, just take a whiz or phone break, or if towards end if shoe, just grind it out.
Now, speaking only for myself - if I have a decent game, am heads up, know that there is a good chance I will remain heads up - I don't care where I am - I'll just grind it out. This is where knowledge of deep negative indices comes in handy. I've ground through lots of shit shoes, losing most hands, to to find myself even, slightly up or down by shoes end. Doesn't take much.
As an add on to my comments.
If you know you can stay heads up, there should be no issues grinding through in negative shoes. Granted, you will lose on these shoes - however, if you're confident of continued heads up play, u get all of the good cards - a good trade off as far as I'm concerned.
Once when I first started playing I was in a casino with only $1000. Family wanted to go, so I went, 2D was open, $25 min. Lost most bets and didn't have the cash to DD on the last play...a juicy split 8 vs a 6. To be clear, I didn't bring a bankroll because I didn't intend on playing, but I learned my lesson.
I leave after I've shown my max bet, so this hasn't happened since. I have lost several running max bets in one shoe to be sure, but it seems natural to leave then anyway. It also seems natural to leave at a big win.
I play for longevity.
If I've had a huge loss or win, leaving also gives me the opportunity to resize my bets for the rest of the trip.

Stop playing when-
You are fatigued, you have shown your top bet, you get heat, your trip bankroll is gone, you don't have enough to start the next shoe, when it gets so crowded that your hands per hour dramatically drop, when you have to be somewhere.
What did I leave out? I didn't mention leaving after winning or losing a certain amount. If you have the advantage keep playing. Have you heard the oft repeated saying that it's one long lifetime session? Think about it. Stop after a win or a loss and you may be leaving money on the table.
