How on earth do you remember all of that? I track the name of the casino, shift, buy-in, results, time played, type of game played, and comments (dealers giving exceptional pen, heat, etc.). I have enough trouble keeping all that straight until I get a chance to write it down.
I have very little recollection of the individual hands played unless something really unusual happens, such as resplitting to 4 hands and doubling on all of them with max bets out.
Those I remember. :-)

I don't keep track of # of Blackjacks, or DDs or Splits, but I do make a note of the casino, total spread, what time of day I played, the pit/floorman, and also who I played with (since I frequently play as a 2-man team), in addition to many of the things you noted above.

While looking over your list, I am curious as to what benefit can be gained by recording a number of the categories you have listed. What does it matter how many DD's or splits are won or lost, min count, max count, or how many BJ's you received during a session?
Here's the things I record:
1 Casino Name
2 Date / Shift / time
3 Name Given
4 DOB Given
5 Description (mine on that day)
6 Clothes / Jewelry worn
7 # Decks / Rules
8 Table min/max
9 Bet Spread
10 Time Played
11 Buy In
12 Cash Out
13 Win/Loss Amount
14 Pen
15 Shuffle Info
16 Dealer Info / name (ie pen, fast, slow, sloppy shuffle etc)
17 Floor Person / Pit Boss Info / name
18 Heat / backoff / barring Info
19 Comp Card Issued
20 Notes
I carry an electronic personal organizer which is password protected and record items 1 thru 9 before I walk in the casion or in a bathroom stall before I even sit down to play. Then after play I immediately record items 10 thru 20 as soon as I leave the casino while it's fresh in my mind.

Your list is very similar to mine Card.
Not as fresh as yours however, (actually that's a great idea) because I record most of my stuff when I get home. I only live an hour, so it's still pretty fresh.
Very frequently I have a room and record my stuff when I get up there.
I'm exceptionally big on pen levels, shifts, heat and the info I supplied them with and the various ID's I have on me.
I also pay particular attention to what i'm wearing, especially rings and watches etc.
I still remember the day I met you in the casino and didn't recognize you because you altered your appearence so much that the only reason I knew it was you was because I recognized your wife lol lol lol.
My goodness,
Look at everyones list,
Hard to believe what we all go through to play this game lol
No wonder they try and throw us out

I don't track anything. If something really important happens, I'll try to remember it, but I don't really write anything down or keep any files.

Keep It Simple Sidekick...
Date
casino
game & rules
minutes played
Won
Cum Win
Win Rate
Remarks (Dealer, pen, shift, etc)
Most important, though, is to track it with a spreadsheet to graph results and isolate certain stores, trips, etc. In my spreadsheet I have separate sheets for results from Tunica, Biloxi, Vegas, AC, etc.
Can't see any pertinent reason to maintain # of blackjacks, buyins, min/max counts??? I have hard enough time to jot down what little info I track, as is.
Guess we all have our reasons.

PM
